Parciální androgenní deficienceu stárnoucích mužů a možnostijejí léčby

Je zcela zřejmé, že narůstající věk je spojen s poklesem produkce několika hormonů. Nejvíce jeto patrné na androgenech, významně se to však týká i dalších hormonů, např. růstovéhohormonu a melatoninu. Klinický syndrom (P)ADAM („[partial] androgen decline in the agingmale“) je sice popsán, ale přímá souvislost mezi projevy tohoto syndromu a změnami specifickýchhormonů není zatím jasně definována. Urologové, internisté i praktičtí lékaři se ve svépraxi často setkávají právě se staršími muži, u kterých je prevalence hypoandrogenních stavůnejvětší. Právě proto by měli být tito lékaři dobře seznámeni s důsledky těchto stavů, s jejichvyšetřováním, léčbou a sledováním účinků léčby. Právě tímto směrem je tato práce zaměřena.

Overview

There is evidence that advancing age is associated with a decline in the production of severalhormones. The most prominent alterations are related to the sex steroids but also otherhormones such as GH and melatonin are affected. The clinical syndrome of (P)ADAM has beendescribed but a direct causality between its manifestations and the alterations in a specifichormone are not yet fully established. The patient population in a urology or GP’s practicefrequently involves individuals beyond the middle age, when hypoandrogenism becomes mostprevalent. It behooves the urologist a GP’s to be familiar with the consequence of this condition,its investigation, treatment and monitoring.
