Možnosti Nimbexu (cisatracuria) ve stomatochirurgii*

Autoři posuzovali použití cisatracuria (Nimbexu) u 21 pacientů operovaných v první polovině roku 2001 pro úrazy obličejového skeletu (zlomeninymandibuly a střední obličejové části). Cisatracurium použili k intubaci u 13 nemocných a současně hodnotili intubační podmínky po 120 sekundách,které byly ve všech případech dobré až excelentní. Pro další vedení anestezie bylo zvoleno u poloviny pacientů kontinuální dávkování cisatracuria 1mck/kg/min, které se osvědčilo u kratších výkonů (do 60 minut), u druhé poloviny bolusové přidávání po 40 a 30 minutách v dávce 0,025–0,03 mg/kg.Extubační podmínky byly ve všech případech dobré (u skupiny s bolusovým podáváním cisatracuria bylo nutno čtyřikrát použít dekurarizaci). Extubacebyla umožněna i v případě naložení mezičelistní fixace. Nebyly pozorovány žádné projevy alergické reakce, ani v podobě kožního erytému.

The authors evaluated cis-atracurium (Nimbex) in 21 patients who underwent general anaesthesia due to facial (mandibular and maxillary) fractures.In 13 patients cis-atracurium was used for intubation and after 120 sec the intubation conditions were evaluated. The conditions were sufficient andalmost excellent in all cases. For maintance of anaesthesia cis-atracurium was used in half of the patients in continuous infusion of 1 µg/kg/min. Thisway was superior for short procedures (60 minutes). In the other half of the patients cis-atracurium was supplemented by boluses of 0.025–0.030 mg/kgat interval of 30–40 minutes. The conditions for extubation were sufficient in all cases (in the group with intermittent supplementation of cis-atracuriumwe had to reverse neuromuscular blockade in 4 patients). Extubation was possible even in the case of intermaxillary fixation. There were no allergicreactions (no cutaneous erythema).
