Fast-track anestezie s užitím alfentanilu pro operace srdečních chlopní

Autoři uvádějí zkušenost s fast-track anestezií s užitím alfentanilu u 22 nemocných, kteří byli operováni pro onemocnění aortální chlopně, vzestupnéaorty a mitrální chlopně. V tomto souboru bylo 8 nemocných ve středním operačním riziku (EuroSCORE 3–5) a 11 nemocných ve vysokém riziku(EuroSCORE>6). Osm pacientů (36,3 %) bylo extubováno 10 minut po skončení operace na operačním sále, u ostatních byla průměrná doba uměléplicní ventilace 51 ± 45 minut. Nikdo z nemocných nebyl reintubován pro respirační nebo kardiální selhání. Průměrná doba pobytu na pooperačníjednotce intenzivní péče činila 2,2 ± 1,4 dnů. Nikdo z pacientů nezemřel do 30 dnů po operaci. Autoři považují techniku fast-track anestezie bez hrudníepidurální analgezie s užitím alfentanilu za vhodnou i pro výkony na mimotělním oběhu u rizikové populace nemocných.

The authors describe their experience with 22 patients who underwent the fast-tract anaesthesia with alfentanil for the lesion of aortic valve,ascending aorta or mitral valve. 8 patients had moderate risk for the surgery (EuroSCORE 3–5) and 11 patients had high risk for the surgery(EuroSCORE>6). 8 patients (36.3%) were extubated 10 minutes after the end of the procedure in the operating room. The average time of mechanicalventilation in other patients was 51±45 minutes. No patient was reintubated for the respiratory or cardiac failure. Average length of stay in the intensivecare unit was 2.2±1.4 days. No patient died in 30 postoperative days. The authors’opinion is that the fast-track anaesthesia with alfentanil and withoutthoracic epidural analgesia is also suitable for cardiac surgery requiring cardiopulmonary bypass in high risk patients.
