Kalcifikujúci sklerotizujúci tumor ovaria: neskoré štádium sklerotizujúcehostromálneho tumoru?

Popísaný je prípad kalcifikujúceho stromálneho tumoru ovaria u 23-ročnej ženy. Tumor nebolasociovaný s hormonálnymi prejavmi ani s Gorlinovým syndrómom. Prominentným histologickýmznakom bola pokročilá sklerotizácia s početnými kalcifikáciami. V architektúre lézie bolanaznačená pseudolobularita. Bunečnatosť bola nízka a tvorená vretenovitými bunkami bez atypiea mitóz. Znázornenie ciev protilátkami anti-CD34 a anti-CD31 ukázalo prekvapujúco vysokú vaskularizáciutumoru, ktorá nebola zreteľná v rutinných histologických rezoch. Imunohistochemickyexprimovali bunky tumoru alfa-inhibín, CD99, alfa-hladkosvalový aktín a vimentín. Vzhľadomna pseudolobulárnu architekturu a bohatú vaskularizáciu tumoru, ktoré sú typické aj pre sklerotizujúcistromálny tumor ovaria, je diskutovaná možnosť histogenetického vzťahu oboch lézií.

Overview

An unusual calcifying tumor of the ovary occurring in a 23-year-old woman is described. Thetumor did not produce any endocrine symptoms. Histologically, it was paucicellular and displayedextensive sclerotic stroma with numerous calcifications. The fibroblast-like cells were arrangedin vague pseudolobules thus resembling overall the architecture of the sclerosing stromal tumor(SST), but the hemangiopericytoma-like vasculature typical of SST was not seen. However, immunohistochemicalendothelial markers CD31 and CD34 showed a surprisingly high number of smallvessels that were not apparent in hematoxylin-eosin stained sections. The tumor cells were positivefor alpha-inhibin, CD99, actin, and negative for desmin, S100 protein, and epitehelial markers.This type of ovarian stromal tumor may represent a calcifying stage of SST.
