Trombelastograf při transplantaci jater, porovnání s konvenčními koagulačními testy

Východisko.
Cílem naší práce bylo během transplantace jater porovnat výsledky získané konvenčními koagulačnímitesty a trombelastografickými nálezy a zjistit jejich význam pro krevní ztráty.Metody a výsledky. U 25 pacientů jsme na konci anhepatické fáze transplantace jater prováděli trombelastografickévyšetření a rutinní laboratorní testy. S krevními ztrátami statisticky významně koreloval pouze protrombinový časa reakční čas R. Tyto dva testy korelovaly též navzájem.Závěry. U našich pacientů se v krevních ztrátách uplatňoval nedostatek faktorů protrombinového komplexu.Trombelastografické vyšetření nenahrazuje při akutní poruše hemostázy konvenční laboratorní testy, ale vhodně jedoplňuje.

Background.
The aim of our study was to compare the results of conventional tests and thromboelastography duringliver transplantation and to determine their importance for blood loss.Methods and Results. Thromboelastography and conventional laboratory tests were undertaken in 25 patients atthe end of the anhepatic phase. Transfusion requirements correlated significantly only with prothrombin time andreaction time, R. These two tests likewise correlated significantly one with the other.Conclusions. Lowered plasma levels of coagulation factors of the prothrombin complex influenced the blood lossesin our patients. While not replacing conventional tests, thromboelastography can serve as an additional test formonitoring acute changes in hemostasis
