Gene Therapy for Liver Tumours: First Clinical Trials
Genová léčba jaterních nádorů: výsledky prvníchklinických studií
Jaterní nádory představují závažný společenský problém a pouze malé procento je vhodné k chirurgické léčbě, kteráje jedinou potenciálně kurabilní. Genová léčba představuje novou strategii, která prokázala dobré preklinickévýsledky v in vitro a in vivo studiích. Použití nádory potlačujícího genu p53, suicidiálních genů, imunogenová léčbaa použití replikačně-kompetentních onkolytických adenovirů vstoupilo u jaterních malignit do fáze prvních klinickýchzkoušek a právě tyto metody jsou v článku rozebírány. U pacientů s nádory jater prokázaly první klinické studieI. a II. fáze dobrou toleranci a nízkou toxicitu genových preparátů, nicméně klinický efekt byl u pacientů po aplikacip53 nebo E1B-deficitního adenoviru nevýznamný.

Only a small percentage of primary and secondary liver tumours is suitable for surgical resection. Gene therapyrepresents a novel strategy that seems to be effective both, in vitro and in vivo. The use of tumour suppressor genep53 therapy, suicide gene therapy, immune gene therapy and therapy with replication-competent oncolytic adenovirusesin liver tumours already entered the first clinical trials. In patients with hepatocellular carcinoma, the firstclinical trials in phase I and II showed good tolerance and low toxicity to gene therapy. However, the clinical benefitfor the patients treated either with wild type p53 or E1B deleted adenoviruses were marginal.
