Karotická mikroendarterektomie
Stenóza vnitřní krkavice na aterosklerotickém podkladě ohrožuje nemocného mozkovou ischémií. Profit z operačníhořešení mají nemocní s vyšším stupněm stenózy, u které je vyšší riziko iktu. Operace je indikována na podkladěklinického stavu a velikosti stenózy. K operaci vhodný stupeň stenózy byl stanoven mezinárodními multicentrickýmirandomizovanými studiemi, takže indikace k operaci je považována za léčebný postup založený na podloženýchdůkazech. Na neurochirurgických pracovištích se provádí karotická mikroendarterektomie, jejíž výhodou je mikrochirurgickátechnika, která umožní dokonalé provedení celého výkonu bez nutnosti použití záplaty a snižuje rizikoembolizace a pooperační disekce cévy.Karotická mikroendarterektomie je jen součástí složité problematiky ischémiemozku a její komplexní terapie.

Atherosclerosis of the internal carotid artery with resulting stenosis, threatens the patients by cerebral ischaemia.Patients with high-grade stenosis, who have a high risk of stroke, gain profit from the surgery. The surgeon’s decisionis based on the clinical picture and on the degree of stenosis. The degree of the stenosis indicated for surgery wasdetermined by international multicentric randomized studies and such indication represents the evidence-basedmedicine. Carotid microendarterectomy is performed at neurosurgical departments. The microsurgical techniqueenables perfect endarterectomy, eliminates patching and reduces risk of embolisation and vessel dissection. Cerebralischaemia is a complex problem and carotid microendarterectomy is a part of the complex management of stroke.
