PREVENCE BAKTERIÁLNÍ ENDOKARDITIDYU NEMOCNÝCH S CHLOPENNÍ NÁHRADOU

American Heart Association vydala v roce 1997 novou verzi doporučení k prevenci bakteriální endokarditidyu rizikových nemocných. Stav po náhradě chlopně řadí nemocného do nejrizikovější skupiny. K infekci dochází přibakteriémiích, jejichž nejčastější příčinou jsou stomatologické a urologické zákroky. Celou škálu lékařských zákrokůpokrývají dva univerzální antibiotické režimy - jeden pro oblast úst, respiračního a horního GI traktu a druhý prooblast urogenitální a dolní GI trakt. V prvním případě jsou to 2 g amoxicilinu p.o. 2 hodiny před výkonem. Výkonyjsou konkrétně uvedeny, vždy jsou také uvedeny případy, kdy atb profylaxe není indikována. Probrány jsou zvláštnísituace.

Overview

American Heart Association published in 1997 new version of recommendations for prevention of bacterialendocarditis in risk patients. Postoperative stadium in patients with prosthetic valve belongs to the highest risk.Infection can develop during bacteremia, which occurs most frequently at stomatologic and urologic interventions.The whole scale of medical interventions can be covered by two universal antibiotical regimes - one for the mouth,respiratory and upper GI tract, second for urogenital and lower GI tract. In the first case 2 g of Amoxiciline areadministered p.o. 2 hours before the intervention. Interventions are individually listed, and cases where antibioticalprophylaxis is not recommended are separately given. Special situations are discussed.
