Venerologická příčina akutní břišní příhody u mladé ženy s rizikovým sexuálním chováním

Sdělení je kazuistikou raritního případu mladé ženy, živící se komerčním sexem, které byla za 2 měsíce po provedené apendektomii v pooperační ráně při následném venerologickém vyšetření prokázána kapavka. Současně byl pozitivní nález gonokoků v endocervixu, uretře a orofaryngu, Peroperačně byl apendix bez známek zánětu. Jde o analýzu mylné interpretace klinického obrazu, způsobů vyšetřování, léčby i jejich nedostatků při stanovení diferenciální diagnózy náhlé příhody břišní. Ve sdělení je také dokumentován rizikový způsob života této ženy, která po všech komplikacích znovu akvírovala kapavku, nepohlavní cestou ji přenesla do hrdla svého dítěte, podruhé otěhotněla a nakonec byla zavražděna klientem při komerčním sexu. Tato kazuistika ve všech souvislostech dokumentuje význam venerologické práce a potřebné mezioborové spolupráce.

Overview

Presented is a case report of a rare case of a young woman making a living through commercial sex in whom, two months after appendectomy, there has been demonstrated the agent Neisseria gonorrhoeae in the posoperative wound. Simultaneously, there was a positive finding of gonococci in the endocervix, urethra, and oropharynx. At the time of surgical intervention the appendix bore no signs of any inflammation. In question is an analysit of an erroneously interpreted clinical picture, of the conducted investigation and treatment and their shortcomings in the differential diagnosis of an acute abdominal event. The report also documents the risky life style of that woman who after all those complications again acquired gonorrhea, through a non-sexual pathway transmitted the infection to the pharynx of her chold, became pregnant for a second time and in the end was murdered by her client in the course of commercial sex. This case report in the whole context documents the importance of venereology and the need of interdisciplinary collaboration.
