Lymphatic mapping and sentinel node biopsy in colorectal cancer
Mapování lymfatik a sentinelová biopsie u kolorektálního karcinomu – možnosti a limity metody
Lymfatické mapování a biopsie sentinelové uzliny je v indikovaných případech široce akceptována pro staging maligního melanomu i karcinomu prsu. Její použití u kolorektálního karcinomu je zatím ve stadiu klinických výzkumů.
Cílem práce je informovat širší odbornou veřejnost o možnostech a limitacích metody, sumarizovat současné zkušenosti v oblasti karcinomu tračníku a konečníku a na základě vlastních i literárních zkušeností stanovit indikační kritéria i zřejmé kontraindikace. V diskusi se autoři krátce zmiňují o aktuálním stavu stagingu kategorie „N“ u kolorektálního karcinomu.
Metoda v sobě nese určitý potenciál ke zpřesnění uzlinového stagingu karcinomu tračníku, předpokladem je dodržení indikačních kritérií, správné technické provedení, dostatečná zkušenost s metodikou a odpovídající laboratorní zázemí.

Overview
Lymphatic mapping and sentinel node biopsy is a widely accepted method for melanoma and breast cancer staging in indicated cases. However, the use of the method in colorectal cancer is under clinical investigation. The aim of the paper is to inform medical professionals about the possibilities and limitations of the technique and to summarize the current state. The actual status of „N“ staging in colorectal cancer is mentioned and shortly discussed by the authors.
The new method contains some potential to precise nodal staging, especially in colon cancer. Correct indications, exact technical performance, sufficient experience and laboratory logistics are essential premises.
Further large prospective studies with standardized techniques are needed to evaluate the potential benefits of this new method.
