Treatment of atrial fibrilation - a clear objectivewith a still obscure resolution
Léčba fibrilace síní - jasný cíls dosud nejasným řešením
Fibrilace síní patří mezi nejrozšířenější arytmii v naší populaci a její výskyt stále stoupá. Jejícelková prevalence je 0,5 %, ale v populaci nad 65 let věku dosahuje 5 % a u nemocných nad 75 letje to až 9 %. Její výskyt u nemocných s kardiovaskulárním onemocněním vede k dvojnásobněvyšší morbiditě a mortalitě a je zároveň nejčastější příčinou cévní mozkové příhody.Na rozdíl od jiných srdečních arytmií nemá léčba FS jednotné schéma a je nutné postupovatindividuálně. Za základ léčby nově zachycených případů s fibrilací síní je stále považovánafarmakologická léčba s dominancí IC, a III. skupiny antiarytmik.Autoři podávají přehled léčby při kontrole komorové odpovědi, možnosti antikoagulační léčby inefarmakologické léčby fibrilace síní.

Atrial fibrilation is of the most frequent arrhythmias in our population and is on the increase.Its overall prevalence is 0.5%, and in the population of aver 65 years of age it is 5%, and in thoseover 75 years even 9%. Its occurrence in patients suffering cardiovascular affections leads toa twofold morbidity and mortality and is the most frequent cause of a cerebral vascular event.Unlike other cardial arrhythmias the treatment of atrial fibrilation has no uniform pattern,and an individual approach is necessary. As a basis of therapy in newly established cases ofatrial fibrilation pharmacological treatment namely with IC and group III anti-arrhythmicalsis preferred.The authors present a review of therapy under check-up of ventricular response, the options ofanticoagulation treatment as well as non-pharmacological treatment of atrial fibrilation.
