Záludnost otrav houbami

Je popsán případ otravy u 53letéhomuže, opakovaně po řadu let konzumujícího „ucháče jedlého“(Gyromitra esculenta), ale ukázalo se, že pacient konzumoval stále čechratku podvinutou(Paxillus involutus). Název ucháč byl regionálních označením. Otrava proběhla pod obrazemmasivní intravazální hemolýzy s rozvojem těžkého renálního selhání, které pacient přežil díkyopakovaným dialýzám a hemoperfuzím. V diskusi se autoři zabývají rozborem otrav oběmazmíněnými houbami a objasňují mechanismus otravy čechratkou.

Overview

The article describes a case of intoxication in a 53 year man who had been repeatedly consuming„turban fungus“ (Gyromitra esculenta) for many years. However, it was discovered that thepatient had been consuming only „brownroll rim“ (Paxillus involutus).The term„turban fungus“was a regionally used name. The intoxication presented itself as a massive intravasal hemolysiswith a developed severe renal failure which the patient survived only thanks to repeatedlyapplied dialyses and hemoperfusions. In the discussion the authors analyse intoxication causedby both fungal species and clarify the mechanism of intoxication with turban fungus.
