Thiazides in the Treatment of Arterial Hypertension
Thiazidy v léčbě arteriální hypertenze
Thiazidy mají dlouholetou zkušeností ověřený spolehlivý účinek na snížení krevmho tlaku. V článku je uveden mechanismus jejich působení v nefronu a předpokládaný mechanismus antihypertenznfho účinku. Saluretický, kaliuretický a sekundárně diuretický účinek thiazidů je provázen nežádoucími metabolickými účinky. Výskyt i závažnost nežádoucích účinků se však snížily při podávání doporučených nízkých dávek (12,5-25 mg/den) při zachování antihypertenzmho působení až do poklesu glomernlární filtrace na hodnotu 0,5 mUs. Léčba Thazidy vyžaduje četnější ambulantní a biochemické kontroly, pro svůj spolehlivý účinek, dobrou toleranci, jednoduché podávání a ekonomickou nenáročnost by však měly zůstat pravidelnou částí antihypertenzní terapie.

Thiazides have a reliable effect on reduction of blond pressure proved by long-term experience. The author describes their mechanistu of action in the nephron and assumed mechanistu of antihypertensive action. The saluretic, kaliuretic and secondary diuretic action of thiazzdds is associated with undesirable metabolic effects. The incidence and severity of undesirable effects however declined after administration of low recommended doses (12.5-25 mg/day) preserving the antihypertensive action up to a dechne of glomernlar filtration to 0.5 mUs. Thiazidy treatment calls for more frequent ambulatory and biochemickl check-up examinations. Because of their reliable effect, good tolerance, simple administration and low price they should remain a regular part of antihypertensive treatment.
