Operace katarakty při silikonovém oleji a jeho vypuštění cestouzadní kapsulorhexe

Vypuštění silikonového oleje po předchozí vitreoretinální operaci je zcela rutinním zákrokem. Často je tato procedura spojena s operací komplikované katarakty.Standardní postup vypuštění oleje ať už v jednom nebo ve dvou krocích předpokládá opětovnou sklerotomii. Současné poznatky i vlastní zkušenosti poukazujína riziko místa sklerotomie.Autoři poukazují na modifikaci vypuštění silikonovéhooleje, při současné operacikatarakty, cestou zadní kapsulorhexe, což umožní odstranění nitrooční tamponády bez sklerotomie a při současné peroperační kontrole zadního pólu.- technika vypuštění silikonu přes zadní kapsulorhexi umožňuje dokonalou peroperační kontrolu stavu zadního segmentu oka- umožní ponechat silikon v oku, je-li nutno prodloužit tamponádu sítnice- snižuje riziko komplikací z další sklerotomie- umožňuje rekonstrukci oka jedním zákrokem a snižuje tak traumatizaci okai pacienta

Elimination of silicone oil after a previous vitreoretinal operation is a routineprocedure. This procedure is frequently associated with a complicated cataractoperation. The standard procedure of oil elimination in one or two stages assumesrepeatedsclerotomy.Contemporaryfindingsandthe authors´ experienceindicatethe risk of the site of sclerotomy. The authors draw attention to a modification ofsilicone oil elimination while operating the cataract by posterior capsulorhexywhich makes it possible to remove the intraocular tamponade without sclerotomyand to implement at the same time peroperative control of the posterior pole.- the technique of elimination of silicone via posterior capsulorhexy makesa perfect peroperative check-up of the state of the posterior eye possible- it makes it possible to leave the silicone in the eye if it is necessary to prolongthe tamponade of the retina- it reduces the risk of complications of further sclerotomy- it makes reconstruction of the eye by a single operation possible and reducesthus the traumatization of the eye and the patient.
