Včasná diagnostika komplikací poranění hrudní stěny a plic – kazuistiky

Autoři se zamýšlejí nad diagnostikou komplikací poranění hrudní stěny. Nejprve uvádějí dvě kazuistiky, ve kterých poukazují na opožděné zjištěnípoúrazového pneumotoraxu a hemotoraxu. V teoretické části poukazují na faktory, které možnost vzniku těchto komplikací zvyšují, a zdůrazňují nutnostkontinuálního sledování pacientů ohrožených těmito komplikacemi. Vedle fyzikálního a laboratorního vyšetření upozorňují především na opakovanáRTG vyšetření hrudníku. Zamýšlejí se rovněž nad indikacemi hrudní drenáže nejen jako terapeutického výkonu, ale i nad její profylaktickou funkcí.

Overview

The authors describe the diagnostic procedures related to complications following thoracic wall trauma. Two cases reports are based on the latediagnosis of posttraumatic pneumothorax and hemothorax, respectively. In the theoretical part, factors increasing the probability of those complicationsare discussed. There is stressed the need for continuous monitoring of patients prone to those complications. The value of repeated chest radiographsis emphasized, along with physical examinations and laboratory tests. Indications for chest drainage for therapeutic intervention are discussed alongwith its prophylactic function.
