Pooperační komplikace perforovaného aneuryzmatu abdominální aorty – kazuistika

V kazuistice 64letého muže je prezentována závažná komplikace po urgentní resekci perforované výdutě subrenální abdominální aorty s přesahem na obě ilické arterie. Při operaci byl proveden aortobifemorální bypass s packingem retroperitonea. Za 3 měsíce po operaci byl pacient revidován pro periprotetický absces s nálezem aortoenterické píštěle, byla provedena sutura píštěle a resekce infikované části bifurkační protézy s náhradou chybějící části novou protézou a byla připojena omentoplastika.

Overview

There discussed casuistics of the 64 years old man, who underwent urgent resection of perforated subrenal aortic aneurysm that expanded to both iliac arteries. Aortobifemoral bypass and packing of retroperitoneum were performed. The patient was operated for periprothetic abscess and aorto-enteric phistula three months later. Phistula was closed, infected prothesis was resected and replaced by new prothesis. Omentoplasty was added.
