Řešení infekce po aorto-femorálních tepenných rekonstrukcích

Infekce umělých cévních náhrad patří k nejzávažnějším komplikacím v cévní chirurgii. Zatím není známa uspokojivá léčba infekce implantátu. Jediným řešením je nadále explantace protézy s těžkými komplikacemi pro pacienta, vedoucími v mnohých případech ke ztrátě končetiny, nebo až k úmrtí postiženého. Předkládaný článek přibližuje na příkladu dvou pacientů náš současný přístup k řešení infekce cévních protéz v aorto-femorální oblasti. Důraz je kladen na zachování cévního zásobení dolních končetin po odstranění infekcí postižené rekonstrukce.

Overview

Arteficial vascular graft infection pertains to the most serious complications in vascular surgery. No satisfactory treatment of implant infection is known yet. The only solution is still explantation of the prosthesis with hard complications for the patient leading in many cases to loss of the limb or even to death of the patient. With the example of two patients the article shows our contemporary approach to solution of vascular prosthetic infection in the aorto-femoral region. The emphasis is put on preserving of the lower extremity vascular supply after removement of the infected reconstruction.
