Indications for Surgical Treatment of the Idiopathic Inflammatory Bowel Disease
Indikace k chirurgické léčbě u idiopatickýchzánětů střevních
Idiopatické záněty střevní jsou onemocněním, které vyžaduje komplexní terapii a v průběhu léčení je často nutnopacienty indikovat k operačnímu výkonu. Přestože konzervativní léčba Crohnovy choroby a ulcerózní kolitidy nemámnoho zásadních odlišností, strategie chirurgické léčby i rozsah operačního výkonu jsou u těchto dvou typůonemocnění výrazně rozdílné. U připraveného pacienta v klinicky i metabolicky dobrém stavu je pooperačnímorbidita i mortalita mnohem menší, proto se snažíme, aby co nejvíce chirurgických výkonů bylo prováděno jakooperace elektivní, i když samozřejmě ne vždy je tento cíl dosažitelný. Kromě správného okamžiku indikacenemocného k operaci je třeba, aby byl i chirurgický výkon proveden ve správném rozsahu, tedy maximálně šetrně,ale přitom dostatečně kurativně a s ohledem na typ a aktivitu onemocnění. Protože vyžaduje značnou zkušenost vesprávný okamžik zvolit správný typ operačního výkonu, chirurgická terapie pacientů s nespecifickými zánětystřevními vyžaduje úzkou spolupráci gastroenterologa a chirurga a měla by být vyhrazena především pro specializovanápracoviště.

Idiopathic inflammatory bowel disease requires complex treatment and patients are often considered to undergo thesurgery in the course of therapy. Though the conservative treatment of the Crohn’s disease and ulcerative colitis hasno major differences, the strategy of surgical treatment and the extent of the operation are in these two diseasesmarkedly different. In an aforethought patient in the clinically and metabolically good shape, the postoperativemorbidity and mortality is much lower. It is therefore desirable to do the most of surgeries as elective, though suchgoal is not always achievable. Beside the appropriate time for the operation, the surgery should be done in the mostproper extent, therefore with the maximal care and considering the type and activity of the disease also with thesufficient curative effect. As the surgical therapy of idiopathic inflammatory bowel diseases requires profoundexperience in order to choose the correct type of operation, the close collaboration between the gastroenterologistand surgeon is necessary and should be performed preferably at specialized departments.
