Transplantace plic – současný stav ve světě a u nás
Transplantace plic se vyvinula ve standardní léčebný postup u pacientů v konečné fázi plicního onemocněnía poskytuje velmi dobré krátkodobé a akceptovatelné dlouhodobé výsledky. Indikace k transplantaci plic jsouchronická obstrukční bronchopulmonální nemoc (CHOPN) a jiné příčiny emfyzému (např. alfa-1-antitrypsin deficience),onemocnění parenchymu (např. idiopatická plicní fibróza, genetické podmíněné nemoci jako cystickáfibróza, vaskulární nemoci (např. primární plicní hypertenze), chronické infekční nemoci (např. bronchiektázie)a některé raritní onemocnění jako lymfangioleiomyomatóza nebo sarkoidóza. Tento článek podává přehled současnéstrategie péče o pacienty po transplantaci plic, chirurgické techniky, limitací a výsledků plicních transplantací.

Lung transplantation has evolved to a standard treatment modality for patients suffering fromend-stage lung diseasesand it provides very good short- and satisfactory long-term survival. Accepted indications for lung transplantationinclude chronic obstructive pulmonary disease (COPD) and other causes of emphysema (i.e., alpha-1-antitrypsindeficiency), parenchymal diseases (i.e., idiopathic pulmonary fibrosis), genetic disorders such as cystic fibrosis,vascular diseases (i.e., primary pulmonary hypertension), chronic infectious diseases (i.e., bronchiectasis), as wellas rare indications such as lymphangioleiomyomatosis or sarcoidosis. Presented article reviews the current strategiesin the treatment of lung transplant recipients, surgical techniques, limitations of and effects of lung transplantation.
