Staging in Patients with Malignant Lymphoma: from Pathological Diagnosis towards Treatment of Particular Patient
Stážovací vyšetření u pacientů s maligním lymfomem: od patologické diagnózy k léčbě konkrétního pacienta
Stážování lymfomů v sobě zahrnuje jednak stanovení oblastí a rozsahu postižení, jednak stanovení dalších prognostických faktorů a faktorů ovlivňujících průběh plánované terapie (věk, komorbidita). Rozsah onemocnění stanovujeme převážně neinvazivními postupy. Většina prognostických faktorů je v současné době tvořena jednoduše dostupnými klinickými a laboratorními markery. Úkolem prognostických systémů či indexů je především určit pacienty, kteří nemají dobrou prognózu při použití konvenční léčby, a jsou tedy kandidáty intenzívních léčebných postupů. Krátce jsou představeny prognostické indexy pro nejčastěji se vyskytující lymfomy - Hodgkinův lymfom (HL), difuzní velkobuněčný lymfom (DLBCL) a folikulární lymfom.

Lymphoma staging includes investigation leading to disclosing of snes of disease and overall disease investigations of other prognostic factors and also of factors influencing the risk of therapy (age, comorbidity). Sites of disease are evaluated predominantly by non-invasive methods. Most of the accepted prognostic factors consist of simple and easily available clinical and laboratory parameters. The main task of prognostic systems is to separate patients with a poor prognosis after conventional treatment, who might benefit from dose-intensive or experimental therapies. The examples of prognostic systems in the three most common lymphhmms - Hodgkin's lymphoma (HL), diffizse large B-cell lymphoma (DLBCL) and follicular lymphoma are shown.
