Germinom pineální a supraselární oblasti u 11letého chlapce

Je popisován případ 11letého chlapce, u něhož byla ve věku 8 let zjištěna hypotyreóza. V 9 letech se u něj náhleobjevily polydipsie, polyurie, bolesti hlavy, diplopie a hypopituitarismus. MRI vyšetření CNS prokázalo dvasamostatné tumory lokalizované v pineální a supraselární oblasti. Histologickým vyšetřením resekované částitumoru pineální oblasti byl diagnostikován germinom. Chlapec podstoupil radioterapii a chemoterapii s navozenímkompletní remise. Za 6 měsíců došlo k prvnímu relapsu nemoci, který byl léčen chemoterapií s indukcí druhékompletní remise. Za dalších 6 měsíců došlo k druhému relapsu. Pro vyčerpání terapeutických možností bylchlapec již léčen jen symptomaticky a při rozsáhlé infiltraci mozku tumorem zemřel na aspirační bronchopneumonii21 měsíců od začátku nemoci.

Overview

The case of 11-year old boy is described. At 8th year of his age a hypothyreosis was found. Polydipsia, polyuria,headache, diplopia and panhypopituitarism occured at 9th year of age. MRI examination of CNS revealed twotumors localized in the pineal and suprasellar regions. Histological finding of resected part of the tumor at pinealregion revealed a germinoma. The boy was treated with radiotherapy and chemotherapy and complete remissionwas induced. Six months later the first relaps arose and the boy was treated with chemotherapy and the secondcomplete remission was induced. After the next six months a second relapse occurred. Because all possibilities oftherapy were exhausted, the therapy was only symptomatic and during the large infiltration of brain by the tumorthe boy died of bronchopneumonia 21 months after the beginning of the disease.
