Efektivita speleoterapie u vybraných respiračných alergickýchochorení detí hodnotená analýzou nákladov zdravotnej poisťovne

Autori predkladajú výsledky ekonomickej analýzy nákladov na lieky a hospitalizáciu u 65 detí s diagnózamiasthma bronchiale (J45) - 36 detí, obštrukčná bronchitída (J41) - 14 detí a iné choroby dýchacích ciest (J31) - 15detí, ktoré v roku 1998 absolvovali turnus speleoterapie (ST) v Bystrianskej jaskyni. Zdravotná poisťovňa hodnotilacelkové náklady na lieky a hospitalizáciu u uvedených detí za kalendárny rok pred ST (rok 1997) a rok po ST(1999). Výsledky preukázali signifikantný pokles počtu dní hospitalizácie a nákladov na hospitalizáciu u všetkýchtroch skupín diagnóz. V ambulantnej preskripcii došlo k signifikantnému poklesu nákladov na lieky u pacientovs diagnózami J41 a J31. U 36 detí s diagnózou asthma bronchiale celkový náklad na lieky stúpol. Toto stúpnutienákladov však bolo u 12 pacientov - u 24 došlo k signifikantnému poklesu. Analýza nákladov u 12 pacientov sovzostupom nákladov zistila, že vzostup nespôsobilo zhoršenie klinického stavu (žiadny z nich nebol hospitalizovaný),ale zmena terapie podľa súčasných štandardov liečby astmy. Pri prepočítaní celkových nákladov došlo k úspore1169,50 Sk na jedno dieťa. Speleoterapia v liečbe alergických a iných respiračných ochorení u detí má nielenzdravotný, ale aj odpovedajúci ekonomický efekt.

The authors submit the results of economic analysis of costs of drugs and hospitalization of 65 children withdiagnoses of asthma bronchiale (J45) - 36 children, obstructive bronchitis (J41) - 14 children and other diseases ofthe respiratory pathways (J31) - 15 children who participated in 1998 in a course of speleotherapy (ST) in theBystrianska cave. The health insurance company assessed the total costs of drugs and hospitalization of thementioned children during the year before ST (1997) and year after ST (1999). The results proved a significantdrop of expenditure on drugs in patients with diagnoses J41 and J31. In 36 children with the diagnosis of asthmabronchiale the expenditure on drugs increased. This increase occurred in 12 patients while in 24 there wasa significant drop. Analysis of costs in 12 patients with increased costs revealed that the rise was not due todeterioration of the clinical condition (none of them were admitted to hospital) but a change of therapy accordingto contemporary standards of asthma therapy. When calculating the overall costs 1 169.50 Slovak crowns per childwere saved. Speleotherapy of allergic and other respiratory diseases in children has not only a medical but alsoeconomic effect.
