Screening of Thyroid Functions in Children and Adolescents with Diabetes Mellitus Type 1


Skríning tyreoidálnych funkcií u detí a adolescentov s diabetesmellitus 1. Typu

Autori sa rozhodli určiť výskyt poruchy funkcie štítnej žľazy v súbore 252 detí a adolescentov sledovanýchv diabetologickom centre SR pri I. detskej klinike LF UK a DFNsP v Bratislave. Vyšetrovali prospektívne 252 detís IDDM, 140 dievčat a 112 chlapcov. 4 deti boli liečené pre primárnu hypotyreózu ešte pred začatím sledovania, 3deti mali okrem diabetes mellitus 1. typu aj Downov syndróm. Rôzne formy tyreopatie diagnostikovali u 18 detí(7,1 %), v 14 prípadoch išlo o novodiagnostikovanú poruchu. Závažná hypotyreóza sa zistila aj u dvoch z trochpacientov, ktorí mali okrem diabetes mellitus aj Downov syndróm. Autori odporúčajú, aby sa robil skríningtyreoidálnych funkcií po manifestácii diabetes mellitus 1. typu a po skompenzovaní metabolizmu. Naprieknormálnym hodnotám tyreoidálnych funkcií pri iniciálnom vyšetrení považujú za potrebné aj ich kontroluv pravidelných dvojročných intervaloch, najmä u pacientov s vysokým rizikom rozvoja tyreopatie, ako napríkladu detí s Downovým syndrómom.

The authors decided to assess the incidence of impaired thyroid function in a group of 252 children andadolescents followed up in the diabetological centre of the Slovak Republic at the First Paediatric Clinic of theComenius Medical Faculty and Paediatric Faculty Hospital in Bratislava. They examined prospectively 252children with IDDM, 140 girls and 112 boys. Four children were treated on account of primary hypothyroidismbefore the beginning of the investigation, three children suffered in addition to type 1 diabetes also from Down’ssyndrome. Various forms of thyropathy were diagnosed in 18 (7.1%), in 14 instances a newly detected disorderwas involved. Severe hypothyroidism was detected also in two of three patients who suffered in addition to diabetesalso from Down’s syndrome. The authors recommend screening of thyroid functions after manifestation of diabetesmellitus type 1 and after compensation of the metabolism. Even in case of normal values of thyroid functions duringthe initial examination they consider it necessary to check it regularly after two-year intervals, in particular inpatients with a high risk of development of thyropathy such as e.g. in children with Down’s syndrome.
