Konformní radioterapie nádorů hlavy a krku

V období od října 2001 do března 2002 bylo na Radioterapeutické a onkologické kliniceFNKVléčeno celkem 9 pacientů s lokoregionálně pokročilými tumory hlavy a krku dávkou minimálně70 Gy technologií 3-D konformní radioterapie při konkomitantní aplikaci amifostinu. U všechpacientů byla zaznamenána kompletní odpověď při akceptovatelné akutní toxicitě. Slizničníakutní reakce hodnotíme jako mírné u 4 pacientů, střední mukozitidu spojenou s ložiskovouepitelolýzou jsme pozorovali u 3 pacientů, u dvou pacientů byly v důsledku mykotické infekcepozorovány závažnější akutní reakce. Vzhledem ke krátké době poléčebného sledování nejsoureferovány pozdní vedlejší účinky a terapeutické výsledky.

Overview

Nine patients with loco-regionally advanced head and neck tumours were treatedbetween October 2001 and March 2002 at the Department of Radiotherapy and Oncology,University Hospital Královské Vinohrady. All patients were irradiated using the 3-D conformalradiotherapy technique. The minimal total dose was 70 Gy. Amiphostine at a dose of 500 mg i.v.per fraction was administered concomitantly. Complete response was recorded in all patientswith acceptable acute toxicity. Acute mucosal reaction was minimal in 4 patients and moderatein 3 others. Severe acute reaction due to mycotic infection was recorded only in 2 patients.Because of the short follow-up, therapeutic outcome and late effects are not referred.
