Submentální intubace – chirurgické a anesteziologické aspekty
Cílem této zprávy z naší praxe je zhodnotit submentální intubaci během operace a v pooperačním období u pacientůs kraniomaxilárním traumatem a korekcemi kraniofaciálních anomálií. V době od června 2001 do května 2002 jsme použilituto submentální endotracheální techniku u 5 operovaných s kraniomaxillárním a faciálním traumatem a u 2 nemocných,u kterých byla provedena korekce kraniofaciálních odchylek; jeden nemocný se syndromem van Dubowitze (Le Fort II,osteotomie a distrakce) a jeden pacient s rozštěpem rtu, alveolu a patra (Le Fort I, osteotomie a distrakce).V souladu s publikovanými zprávami se ukázala submentální intubace jako rychlé a spolehlivé zajištění průchodnostidýchacích cest během operace u případů s těžkým mandibulárním, kraniomaxillofaciálním traumatem a u korekcí kraniofaciálníchanomálií. U našich nemocných nevznikly žádné komplikace, které by souvisely s použitou intubační technikou.Tento způsob zajištění dýchacích cest umožňuje chirurgovi optimální přehled operační rány a optimální podmínky prokontrolu okluze chrupu. Je ovšem třeba předem plánovat souhru anesteziologa a chirurga.

The aim of this report from our practice is to evaluate the submental intubation during procedures and postoperatively inpatients with craniomaxillofacial trauma and corrections of craniofacial anomalies. Between June 2001 and May 2002 thesubmental endotracheal intubation was performed in 5 patients with craniomaxillofacial trauma and in 2 patients withcraniofacial anomalies, the Dubowitz syndrome (Le Fort II osteotomy and distraction) and a patient known with a doublecleft lip alveolus and palate (Le Fort I osteotomy and distraction).The impression is that the submental intubation is a handy,fast and safe technique, which provides an airway in cases of severe mandibular and craniomaxillofacial trauma and incorrections of craniofacial anomalies. It enables the surgeon an excellent view and access to of the operation field andpermits optimal intra-operative control of the occlusion. No post-operative intubation related complications were seen.
