Maligní hypertermie
Maligní hypertermie je jednou ze závažných komplikací celkové anestezie. Jedná se o geneticky podmíněnou dispozicis autozomálně dominantním způsobem přenosu na další generaci. Její rozvoj je podmíněn expozicí tzv. spouštěcím látkám,což jsou volatilní anestetika a depolarizující svalová relaxancia typu sukcinylcholinu. Podstatou je porucha regulacekalciového metabolismu na úrovni receptorů sarkoplasmatického retikula svalové buňky. Toto sdělení podává souhrnnouinformaci o historii a současných poznatcích o patofyziologii, genetice, klinickém obrazu a terapii maligní hypertermie.Uvádí důležité údaje pro anesteziologickou praxi, shrnuje způsoby a možnosti současné presymptomatické diagnostikya informuje o stavu diagnostiky ve světě a v České republice.

Malignant hyperthermia is a serious complication of general anaesthesia. It is a genetically determined disposition with anautosomal dominant transfer. The development of MH is conditioned by the exposure to triggering agents – inhalationanaesthetics and suxamethonium.The nature ofMHis a disorder of calcium metabolismin sarcoplasmic reticulum receptorsin skeletal muscle. The review presents an overview of the history and current knowledge of pathophysiology, genetics,clinical picture and treatment of MH, gives important information for anaesthesia practice, summarizes current means ofpre-symptomatic testing and informs on the state of MH diagnostics world-wide and in the Czech Republic.
