Letální intoxikace deriváty amfetaminu

Užívání návykových látek se stále rozšiřuje. Pervitin (metamfetamin) a ostatní stimulancia jsou nejčastěji užívanými "tvrdými" drogami u nás.Závažná intoxikace deriváty amfetaminu může probíhat až za příznaků hyperpyrexie, rabdomyolýzy, akutní renální insuficience, koagulopatiea multiorgánového selhání. Role dantrolenu v léčbě hypertermie u těchto otrav je diskutována. Přesto je zřejmě vhodné jeho časné podání, a to jakosoučást maximálně intenzivní léčby u intoxikací, které se v naprosté většině vyskytují u mladých lidí. Součástí léčby syndromu DIC by mělo být i časnépodání antitrombinu III.

Drug abuse becomes more common than ever before. Pervitin (methamphetamine) and other stimulants belong to the most abused „hard“ drugsin our country. Severe intoxication with amphetamine derivates may present as hyperpyrexia, rhabdomyolysis, acute renal insufficiency, coagulopathyand multiorgan failure. Dantrolene administration in those cases is discussed. Considering the pros and cons, early administration is probably advisableas a part of maximally intensive care for a young intoxicated patient. Early administration of antithrombin III should be a part of the DIC treatmentprotocol.
