Interstitial lung processes (IPP) represent a heterogeneous group of non-infectious inflammatory diseases that can lead to fibrotic reconstruction of the lungs, resulting in irreversible damage and eventually chronic respiratory failure. In the following interactive case study, we present the case of a patient with severe comorbidities, particularly multiple myeloma, who developed IPP with a progressive phenotype.

The diagnosis of idiopathic pulmonary fibrosis (IPF) must be based on the synthesis of clinical and radiological findings and supported by functional lung examination. At the same time, the presence of other interstitial lung processes must be excluded. Hence, it cannot be confirmed during a single visit, often it is the result of deliberation by a multidisciplinary team. Without this, effective antifibrotic treatment cannot be initiated. Systemic scleroderma (SSc) is a rare chronic systemic connective tissue disease. It affects the skin, blood vessels, and several internal organs, often leading to sclerosis and fibrosis. Treatment must be comprehensive and include regular check-ups to detect internal organ involvement and the risk of disease progression early. Recommended lung involvement screening is conducted at regional lung clinics or centers specializing in diagnosing and treating interstitial lung processes (IPP). 1 Our interactive case study details the approach to managing a patient with SSc and the process for diagnosing progressive IPP. The case of a 57-year-old woman with systemic scleroderma associated with IPP is handled by specialists from a regional rheumatology clinic, lung clinic, and IPP center at the University Hospital Olomouc.
